Keeping your Medical Records Updated

If you have changed your name, address or contact details (telephone/mobile number, email address) then please contact us so that we can update your records accordingly. It is important that your records are kept updated to ensure that we are able to contact you should we need to discuss anything. 

If you have access to our online service then you can login and update your records that way. Alternatively you can fill in the form below. Please remember to provide us with your full name (including old name if this has changed), date of birth and full address (again including your old address if this has changed). To complete this form you will need to enter a valid email address which we will add to your records.
