Permanent Registration (PRF1 Form)
To register online please click on the link below to be directed to the NHS website. Please ensure that the forms are filled in correctly; failure to do so may result in your registration being delayed. Once completed we will send a welcome letter in the post outlining who your registered GP is.

If you would like to sign up for online access then please come in and speak to a receptionist. Remember to provide a form of photo identification (eg driving license, passport) as well as confirmation of your address (eg bank statement, tenancy agreement).
Temporary Registration (GMS3 Form)
If you are temporarily staying in the Wisbech area and require treatment during your stay then we also offer a temporary service where you can register here for a maximum of 3 months. Please come into the practice and speak to one of our receptionists for more information.
