Requesting a Sick Note

If you require a new or continuing sick note then please click on the button below to be redirected to our online Klinik service where you will be able to submit a request. Remember that you can legally self certify for the first 7 days of sickness meaning that you do not need a sick note for this period. 


Please ensure that your current sick note has expired before requesting a new one as we will not be able to renew it until your current one expires. Once received we will process your request and send it over to you electronically within 2 working days; please ensure that the email address entered is correct.

Restricted access to the rear car park

There was an incident over the weekend where part of the wall in our rear car park collapsed. The rubble has been cleared however, to ensure both patient and staff safety it has been decided to restrict access for cars to enter the car park. Patients can still walk in and enter via the rear entrance however they will need to park in the Chapel Road car park. One of the gates has been closed and barriers have been erected to guide patients around the hazard until we are able to have the wall fully inspected and repaired.
