Requesting a sick note

If you require a new or continuing sick note then please fill in the form below. Remember that you can legally self certify for the first 7 days of sickness meaning that you do not need a sick note for this period. Please ensure that your current sick note has expired before requesting a new one as we will not be able to renew it until your current one expires. Once received we will process your request and send it over to you electronically within 2 working days; please ensure that the email address entered is correct.
